Service Roles

The Home Health Team

Physician

The physician who remains the patient’s primary doctor and participates in the Home Health plan of care.

RN Case Manager

The Registered Nurse Case Manager conducts the initial admission visit; established the plan of care for the patient; supervise, and evaluates all health professionals responsible for direct client care. Provides direct home nursing care in accordance with the plan of care signed by the physician. Confers with and acts as liaison to the patient’s physician and all health professionals responsible for patient care.

Field Nurse

The Field Nurse provides in home skilled nursing visits per the established plan of care which include but are not limited to assessing patient needs, direct hand on care, and teaching patient disease processes and medications. The Field Nurse reposts directly to the RN Case Manager to assure the highest quality of care is provided to the patient.

Certified Home Health Aide

The Certified home Health Aide (CHHA) works under the direction of the RN Case Manager in the provision of home health aide services to maximize the comfort and health of patients and families. Maintain personal hygiene and a safe, healthy environment for the patient per the plan of care as established by the RN Case Manager.
